Annual Reporting Form

The diocesan Annual Reporting Form is due April 1 each year. This report includes contact information for lay leadership and ministry leaders (paid and volunteer), certification of elected lay delegates to convention, and other information not reported on the Parochial Report. (For diocesan staff use only.)

4 Delegate certification for the 150th Diocesan Convention

List each lay person who has been elected to represent the congregation at the diocesan annual convention.
Parishes are entitled to four (4) lay delegates and up to 4 alternates. Mission congregations are entitled to two (2) lay delegates and up to 2 alternates.
This information serves as certification of your lay delegates, in accordance with Canon III, Section 5.
